GUEST STUDENT WAIVER

I acknowledge and am fully cognizant of all the inherent dangers in connection with the execution of martial arts and acknowledge that the execution of martial arts requires physical exertion and contact, and realize that there is risk of injury which may be incurred while engaged in such activities. I hereby waive any and all claims for any injury in connection with the activities with Franklin Martial Arts, or Bruno Souza, and assume the risk of all dangers inherent to practicing martial arts.
